Complete Online Health History Intake Form
Complete the Online Health History Intake Form at least 24 hours prior to your appointment time.
Step 3
Read “GLP-1” and “Read prior to giving 1st”
Read the educational material about GLP-1 agonists found in your documents folder titled “GLP-1” and “Read prior to giving 1st…”. You will receive an email notification when these documents are available for review. You may want to write down some questions you have for Dr. Heaton
Step 4
Meet with Dr. Heaton
Login to your GLP1MedRx account at you appointment time to meet with Dr. Heaton and discuss weight loss goals along with treatment options. If everything checks out a prescription will forwarded to the compounding pharmacy.
Step 5
Enjoy Your Healthy New Life
Check your email for information from the compounding pharmacy for registration instructions and payment authorization. Within 2-3 days of your credit card or Health Spending Account (HSA) being charged there should be a package on your doorstep containing the medication, icepacks, and supplies.
